Music Schools International Langley (“MSIL”) recognizes that affording the opportunity for students to perform in a music recital enhances and adds value to its musical educational program by offering students an additional opportunity for music performance to support students’ growth in their emotional, social, and mental development.

This online form MUST be signed and submitted to the office at least 30 days before the Recital dates of May 30th, June 3rd or June 6th, 2026.

I agree to my child’s participation or my participation as an adult (19+) in the musical recitals at Surrey Arts Center on May 30th, June 3rd, June 6th, 2026 (the “Recital”).

In consideration of MSIL offering my child an opportunity to participate in the Recital, the receipt and sufficiency of which as consideration is acknowledged, I agree to waive and release any and all claims, actions, demands, damages, costs, expenses or compensation, I, or my child, may have against MSIL and its employees, agents, or volunteers for any personal injury, damage or loss sustained by myself or my child resulting or arising from or in any way connected to my child’s attendance and participation in the Recital.

I further agree and understand accidents can occur with or without any fault on either the part of the child, or MSIL or the facility where the Recital is taking place. By allowing my child to participate in this activity, I agree to accept the risk of an accident occurring, and agree that this activity, as described above, is suitable for my child’s attendance and participation.

In signing this Consent and Waiver, I am not relying on any oral or written representation or statements made by MSIL to induce me to permit my child to attend the Recital, other than those set out in this Consent and Waiver.

If any provision of this Consent and Waiver is found by a court of competent jurisdiction to be illegal, unreasonable, invalid or unenforceable in whole or in part, the illegality, unreasonableness, invalidity or unenforceability of that provision, shall be reduced, severed, read down or otherwise modified to the extent the said provision(s) are allowable under the law, and the remaining provision(s) shall be effective to the fullest extent permissible.

A electronic executed copy of this Consent and Waiver will have the same force and effect as an original.

I am 19 years of age or more and have read and understand the terms of this Consent and Waiver, and understand that it is binding upon me.

Note: This Consent and Waiver must be signed by ALL custodial parents or guardians of a child who is under the age of 19 years.
